Health Services K-6th Grade
I thought perhpas it might be helpful to outline and explain the services provided to students in Kindergarten through 6th grade, in the Health Office here at Sol Feinstone, and district wide. Many of these services and screenings are required and guided by state law in Pennsylvania, and are state mandates. As such, there are specific guidelines for pass and failure, and for referral to specialists and follow up.
Near and distance vision is checked yearly Kindergarten through 6th grade. Color vision is checked in 2nd grade. If a student fails a screening, the parent is notified, and a vision referral form is sent home, for a vision care specialist (opthamologist or optometrist) to complete. This is a state mandate.
Hearing is checked on all students in Kindergarten through 3rd grade, 7th grade and 10th grade. However, if a student has a known hearing loss, or has failed hearing screenings in the past, the hearing is checked yearly through 6th grade. If a student fails the screening, the parent is notified, and a hearing referral form is sent home for a hearing physician to complete. This is a state mandate.
The state requires that height and weight be checked yearly in Kindergarten through 12th grade. Also, the BMI (Body Mass Index) is calculated yearly on each student. The BMI is a numerical health indicator, regarding height and weight. The BMI information is shared with parents yearly, in a Health Report Card for each student. Out of respect for privacy and sensitivity, the nurse does this screening, no numbers are called out or shared. Height/weight and BMI are state mandates, however, parents may 'opt out' of the BMI notification process, by sending a letter to the Health Office to request this.
Scoliosis checks are required in 6th grade and 7th grade. The 6th grade scoliosis documentation is taken from the child's private physician's notes, on the 6th grade physcial exam form. And, the 7th grade scoliosis check is done by the school nurse and the gym teacher in middle school. This is a state mandate.
Dental Exam Forms
Dental exams are mandated by state law for the following grades; Kindergarten or 1st grade (the year of entry to school), for 3rd grade, and for students who transfer from out of state. ***New last year and continuing-by April 1st, if the Health Office is missing a dental form required for a child's grade level, field trip priviledges and witholding of the final report card, may occur. Won't you please help us, and your child, by promptly sending in these forms? For your convenience, a supply of dental exam form is available in the hallway outside the Health Office, and may also be accessed online. Please go to the district website at www.crsd.org, then click 'Kindergarten and Student Registration', and then click on the form needed. This is a one page form.
Physical Exam Forms
Physical exam forms are mandated by state law for the following grades; for Kindergarten or 1st grade (the year of entry), for 6th grade, and for out of state transfers. ***New last year and continuing, by April 1st, if the Health Office is missing a physical exam required for a child's grade level, field trip priviledges and witholding of the final report cards may occur. Won't you please assist us, and your child, by promptly sending in the required forms?
