8/29/11 Dear Parents and Guardians, Would you kindly note the following information about the Pennsylvania state requirement for physical and dental exam forms? Reminder letters were sent home with students in June. these letters informed parents of the state requirement for a physical or dental that is due the following school year. ****Parents may send in 1)the actual PE or dental form, OR, 2)for your convenience, you may sent or email a note that states the complete date (day, month, year) of the child's last physical or dental exam, and the doctor or dentist's name. **Kindly note, if forms are not received by the last day of school, this may result in the child's final report card being witheld in June. Physical Exam forms are due in September of the Kindergarten or 1st grade year (the year of entry to school), in 6th grade, and for all out-of-state transfers. This is a Pennsylvania state requirement. Forms are available on the shelves located immediately outside the Health Office, or you may download the two-page form, by accessing the CRSD link below. Dental Examination forms are due in September of the Kindergarten or 1st grade year (the year of entry to school), in 3rd grade and for all out-of-state transfers. This is a Pennsylvania state requirement. Please note that in August, the fax number at school was changed.
